Overview
We’re seeking a compassionate, licensed clinician to provide essential support and consultation to the Outreach team within our Community Support Program for Homeless Individuals (CSP-HI). This role involves guiding case managers, coordinating client care, and ensuring compliance with state regulations. Qualifications
- Licensure: Massachusetts-licensed clinician (LCSW, LICSW, LMHC)
- Experience: 1-2 years in geriatrics and mental health; experience with homelessness and substance use preferred
- Skills: Proficiency in supporting adults with behavioral health needs and navigating multi-agency collaboration
Responsibilities
Clinical Support & Case Management:
- Provide guidance and consultation to case managers on client issues, including screenings, assessments, and action planning.
- Oversee individualized action plans, collaborating with external providers to ensure comprehensive support.
- Conduct regular observations and evaluations to support and train case managers, enhancing their clinical skills.
Program Compliance & Administration:
- Ensure all documentation aligns with CSP-HI and Massachusetts Medicaid requirements.
- Manage weekly CSP-HI billing, coordinating with Finance and Eliot Human Services to meet submission deadlines.
- Participate in interdisciplinary team meetings to discuss case management practices and support agency policy development.
Crisis Intervention & On-Call Support:
- Provide direct intervention in behavioral health crises, offering support to both clients and case managers.
- Share rotating on-call responsibilities, including nights and weekends, to address urgent client needs if needed.
- Conduct ongoing risk assessments and ensure follow-up care with external agencies when needed.
Staff Training & Development:
- Lead trainings on client confidentiality, crisis management, and clinical documentation best practices.
- Coordinate interdepartmental training on topics impacting the wider Outreach team.
- Offer clinical consultation and supervision to MSW interns, supporting their professional growth.
Additional Responsibilities:
- Participate in team meetings and multi-disciplinary discussions, contributing to program enhancements.
- Stay current on CSP-HI documentation protocols and attend relevant training sessions.
