North Suffolk Community Services Hiring Senior Outreach Worker

Location: Chelsea, MA

Job Description:
NSCS is looking for a Senior Outreach Worker to join our Community Supports Program (CSP)! The Senior Outreach Worker is a supervisory position that provides supervision and training to CSP Outreach Workers. This position supports Outreach Workers in providing a range of mobile, care coordination services to assist adults with resources needed to live successfully in the community. Services may include advocacy, coordination of treatment, outreach, assistance with basic needs, skill building and facilitating linkages with other community based and natural support systems.

Work demand is split: mobile and remote. Primary mobile when serving consumers who live in various urban community settings in the Boston proper area. Remote when providing supervision to CSP Outreach Workers.

What You’ll Do:

Provides weekly supervision to all Community Supports Program Outreach Workers.
Approves EHR progress notes submitted by CSP Outreach Workers to ensure standards identified by funders.
Assist CSP Program Manager with staff training.
Assist CSP Program Manager with new marketing initiatives.
Acts as a liaison with community providers.
Assist with the assignment of referrals to the CSP Outreach Workers.
Reviews and ensure productivity standards are met by Outreach Workers.
Maintains a caseload based on the amount of supervisees where the following will be provided to the

Client:
Establishes and maintains professional relationship with the client, providing intensive case management services.
Acts as a liaison between service providers, community and client.
Supports in connecting client with needed community resources such as but not limited to:
entitlement programs,
budgeting support,
housing assistance,
community integration,
medical and or behavioral health supports,
transportation needs,
recovery supports and other psychosocial need areas.

What You’ll Need:

Education:
Master’s level degree in social work OR a related field.

Experience:
Previous outreach experience with populations with mental illness and/or substance abuse required.
Proven ability to collaborate effectively with other providers.
Supervisory experience is preferred.

Skills:
Must possess a valid Mass driver’s license and reliable transportation.
Strong communication and liaison skills
Capable of intervening/ assessing during crisis situations.
Good oral and written skills required.
