MetroWest Free Medical Program hiring for Case Manager Coordinator

Location Framingham, MA

The MetroWest Free Medical Program has been providing free care to uninsured and underinsured patients since 2004. Our services include Adult Medicine, Women’s Health, and Vision Care and we deliver care through a combination of in-person services and virtual care. We are a critical part of the patient safety net in the area West of Boston. Most of our patients are non-English, Portuguese speaking and have difficulty accessing care. We provide a warm, accessible entry point to the health care system.

The Case Manager Coordinator will work to help patients at the MetroWest Free Medical Program navigate financial, social and organizational aspects of healthcare. This will include connecting uninsured patients with health insurance and helping to refer patients to a primary care provider or other necessary/helpful resources. 

The coordinator will be responsible for building, training and leading a capable team of certified application counselors, case management, and patient navigator volunteers who will aid patients in the aforementioned goals. In addition, the coordinator will work to identify possible primary care, behavioral health and pharmacy partners for the MetroWest Free Medical Program. 

This position will also involve creation of a standardized patient navigation form, to be integrated as part of the EHR, that will help collect data on the number of patients who were already insured, were helped to register for insurance and/or referred to a practice.

The Coordinator will coordinate efforts with the three clinics, working collaboratively with the clinical managers and IT volunteers. The Patient Navigation Manager will prepare periodic reports to the Executive Director in order to support grant proposal development and reports to funders.

Requirements/Qualifications *
Bachelor’s Degree in medical or health profession
Volunteer or paid experience working in a health care or social services setting.
Commitment to mission, culturally-responsive practice and health equity
In-depth knowledge of health and safety guidelines and procedures and willingness to follow them at all times
A team player with excellent communication and interpersonal skills
Responsible and compassionate
Outstanding organizational and multi-tasking skills
Patient, with excellent problem-solving skills
Experience working with interpreters and a diverse patient and staff population
Knowledge of local community resources or ability and willingness to access these
Willingness to comply with MWFMP policies and procedures
Portuguese and/or Spanish fluency a plus
