Full time (1.0) Senior Social Work position in Adult Palliative Care — NYU Langone, Manhattan campus

This position is an integral member of the interdisciplinary Palliative Care consulting team, which aids in the development of palliative care plans for patients and families facing serious illness and end of life in all hospital departments.  The palliative care social worker coordinates with physicians, nurse practitioners, nurses, chaplains, and the unit based care team that includes care managers and social workers to expedite appropriate, safe and timely post-acute care plans.  The palliative care social worker provides direct clinical counseling and crisis/grief interventions to patients and families, and will facilitate staff debriefs. 

Responsibilities include, but are not limited to, comprehensive psychosocial assessment to assess individual and family coping strengths, understanding of medical information as it relates to end stage illness or end of life, development of interventions to address barriers and needs, participation in HRO huddles to help engage and integrate unit staff on care plans and best practices, provide supportive counseling to promote patient and family adjustment to illness, catastrophic diagnoses, anticipatory grief and bereavement, perform data collection as part of Joint Commission Accreditation standards and participate in committee level work to enhance the provision of Palliative Care supports across the continuum. 

Potential candidates should have strong interest in working with patients and families at end stages of illness, and with those living with chronic and progressive disease, be comfortable and have therapeutic knowledge to help patients and families through the dying process, and have interest/experience in program development to partner with leadership to implement an adult bereavement program.  The position requires an MSW degree, to hold an LCSW (or complete prior to end of 6 month probation), at least 4 years of experience as a licensed social worker and completion of a Palliative/End of Life Care certificate or fellowship from an accredited program.  The position will be 5 days a week.
