Responsibilities

PRINCIPAL DUTIES AND RESPONSIBILITES:
The incumbent performs specialized treatment of complex mental illness for patients seen in the SARRTP. SARRTP residents present with a wide variety of healthcare needs including substance use history, mental health problems, and medical issues. The incumbent practices at the independent level. The Social Worker provides assessment and treatment of Veteran patients within the approved scope of practice. Typical duties are listed below; however, incumbent willingly accepts additional and special assignments that are necessary to meet program goals or customer needs.

Screening and Assessment: Interviews Veterans and their family members/significant others to establish facts about the Veteran’s situation, presenting problems and their causes, and the impact of such problems on the Veteran’s functioning and health as part of a comprehensive psychosocial assessment. Reviews all data, subjective and objective, and makes a clinical assessment identifying strengths, needs, abilities, and preferences. Uses advanced clinical training, insight, and experience to interpret data and to identify viable treatment options. Assesses high risk factors, acuity, and need for services.

Treatment Planning: Develops psychosocial treatment plans in coordination with interdisciplinary team members, including goals for psychosocial clinical treatment. Using known available resources and the initial assessment of the Veteran’s likelihood to accept differing types of assistance, makes initial and continuing decisions regarding use of VA and non-VA services and referrals. Works collaboratively with the Veteran, taking into consideration the Veteran’s self determination, when presenting VA and community-based referral options for the Veteran’s needs/goals. Attends interdisciplinary treatment team meetings, updating treatment and discharge plans as appropriate.

Treatment Implementation: Serves as the social worker and case manager for Veterans who are in the SARRTP. Provides clinical services to Veterans and family members/significant others in support of the Veteran’s treatment. Provides individual and group counseling services, including evidence-based therapies. Employs Social Work methods and techniques to maintain a therapeutic relationship that will help the Veteran work toward the best way of achieving their goals. Maintains a current and productive network of referral resources. Makes referrals to other services in the VA, as well as to other agencies, community resources, and other Governmental agencies. Receives and completes requests for services (consults) from interdisciplinary team members and from other Social Workers on complex, difficult cases, using advanced practice skills and expertise. Advocate for resources to assist the Veteran in meeting their needs. Utilizes crisis intervention techniques with Veterans who are in an acute crisis, and knows and implements the Suicide Prevention and Management of Suicidal Behavior policy.

Discharge Planning: In collaboration with the Interdisciplinary team, develops discharge plans and sets achievable discharge goals with consideration of the Veteran’s self-determination and personalized recovery goals. Coordinates post-discharge services, including VA services and community-based services. Establishes effective working relationships with VA staff and community-based agencies.

Evaluation, Development, Education, Supervision, Coaching, and Mentoring: Takes a supportive role in the administrative/clinical functions of the program such as quality assurance, performance improvement, program development, policy and procedure development, preparation of reports, preparations for surveys, and other duties as assigned. Directs activities to maximize effectiveness, efficiency, and continuity of care for Veterans. Provides training to staff through orientations, patient review conferences, and in-service programs. Serves as a mentor to other Social Workers, particularly those at the entry level, and provides clinical supervision for unlicensed Social Workers. Supervises Social Work graduate students and provides a training experience for them. Provides training to students, residents, interns, and fellows from other disciplines. Keeps abreast of new developments in the field of substance abuse and mental health.

Documentation and Reporting Requirements: Documents independently in the medical records, including psychosocial assessments, diagnostics, treatment, progress notes, follow up, and referral. This information will be entered in a complete, confidential, and professional manner. Complete appropriate clinical reminders and capture all caseload activity.

Work Schedule: 8:00 AM to 4:30 PM, Monday through Friday
