Geriatric Social Worker/Case Manager

Geriatric Social Worker/ Case Manager. Full-time salaried position. Candidate should be self confident & responsible with strong advocacy, assessment, problem solving & communication skills. Case manager provides interventions to help older adults at risk for abuse, exploitation or neglect live as independently as possible. This work makes a real difference in the lives of vulnerable elders. Collegial and supportive working environment. Excellent benefits package includes 4 weeks paid vacation. LICSW supervision provided. 

Essential Job Functions:

  • Works as part of case management team to provide interventions to alleviate factors placing the elder at risk and maximize quality of life.
  • Works collaboratively with Elder Protective Services, other community agencies, medical and personal care providers. 
  • Consults with legal counsel and attends court hearings.
  • Provides a broad range of services including applying for benefit programs, obtaining health care, and exploring and arranging for most appropriate, least restrictive living environment.
  • Visits to monitor status of clients living at home, in assisted living and long term care facilities in Greater Boston area.
  • Monitors and oversees medical & personal care for caseload; provides advocacy.
  • Responsible for maintaining client files, progress notes and other documentation

Requirements/Qualifications

  • MSW preferred
  • Experience with geriatrics and dementia preferred.
  • Candidate must have car.
  • Knowledge of Microsoft Office Suite (excel, word, outlook, power point)

Salary 

$44,000
